Instructions :
The following documents are required:
1. BIRTH CERTIFICATE(ORIGINAL AND SELF-ATTESTED PHOTOCOPY)
2. PROOF OF RESIDENCE
- RATION CARD/SMART CARD ISSUED IN THE NAME OF PARENTS (MOTHER/FATHER HAVING NAME OF CHILD).
- DOMICILE CERTIFICATE OF CHILD OR OF HIS/HER PARENTS.
- VOTER LI-CARD (EPIC) OF ANY OF THE PARENTS.
- ELECTRICITY BILL/MTNL TELEPHONE BILL/WATER BILL/PASSPORT IN THE NAME OF ANY OF THE PARENTS OR CHILD.
- APPLICANT’S AADHAR CARD.
3 IDENTITY CARD OF THE ELDER CHILD FOR THE SIBILING CATEGORY.
4 MARK SHEET OR PASSING CERTIFICATE FOR THE ALUMNI PARENT CATEGORY.
5. PASSPORT SIZE PHOTOGRAPH OF CANDIDATE, MOTHER AND FATHER
6 COPY OF GOOGLE MAP FOR DISTANCE.
Note : The Registration will be treated as provisional unless the hardcopy of the filled form
with supporting documents is not submitted within next three working days.
